Reservation Form — High Holy Days 2010/5771


Temple Beth Tikvah’s High Holy Days services are open to everyone and are free of charge for TBT members. If you plan to attend, please complete the registration form below.

Those who are not current members may attend any or all of the High Holy Days services for a minimum donation of $50 per adult or $100 per family. Of course, additional donations in honor of the High Holy Days from members and non-members are always appreciated! Please send a check to "Temple Beth Tikvah", P.O. Box 7472, Bend, OR 97708. (If the donation request presents a financial hardship, please contact our Treasurer, Jeff Adler at jadler@bendcable.com to make special arrangements. Your request will be kept strictly confidential.)

Non-members who choose to join Temple Beth Tikvah in 2010, will have their High Holy Days donation amount applied toward membership. We warmly welcome your participation.

Only one reservation per family is needed. If you have any questions, please call
541-388-8826 and leave a message with your name and phone number. A representative of Temple Beth Tikvah will return your call.

Please include the full name used on your registration form in the memo field of your check. One check can be used to pay for Break Fast reservations, names for the Yizkor booklet, and High Holy Days donation.

If you would like to have names of departed loved ones printed in our Yizkor Memorial Booklet, please enter them below. Please indicate if someone passed away after Sept 18, 2009 as those names will be read aloud during the Yizkor service. A donation charge of $36 entitles you to as many names as you wish. Names for the booklet must be received by Aug 31 to appear in the booklet.
